Skilled Nursing

A skilled nurse is an RN or LPN, trained to provide nursing services in your home, under MD orders, to help aide in your recovery. The nurse will lead your health care team in the coordination of your care and will help to increase your knowledge regarding your diagnosis, medications and treatment. Your nurse is in frequent communication with you and with your MD, to ensure that your plan of treatment is up to date and relevant to your current health condition.

Below are services that our Skilled Nurses will provide include, but are not limited to:

  • Symptom Management

  • Cardiovascular Disease Care

  • Diabetic Care

  • GI Tube Management and Care

  • PT with INR Checks

  • Medication Education

  • Wound Care

  • Incontinent Care

Resistance Band

Physical Therapy

Our Physical Therapist or Physical Therapist Assistant will evaluate and treat you, if you are suffering from a physical disability due to injury, disease or surgery. The Physical Therapist will monitor, document and report your progress, based on the treatment schedule ordered by your physician.

Below are services that our Physical Therapist will provide include, but are not limited to:

  • Muscle Toning and Coordination

  • Range of Motion Exercises

  • Muscle Strengthening

  • Energy Conservation

  • Orthopedic Therapy

  • Home Safety Assessment and Instruction

  • Assessment and Treatment of Pain

  • Patient/Caregiver Education

  • Wheelchair Assessments

  • Assessment, Treatment and Training with assistive ambulation devices, ADL Adaptive Equipment, etc.

  • Instruction and Management of Home Exercise Programs, Gait Training, etc.

Occupational Therapy

The Occupational Therapist or Occupational Therapist Assistant will create an individualized occupational therapy program for the patient after reviewing your physicians orders.   In the comfort of your home, our Occupational Therapist will assist you in, regaining your ability to perform daily activities. The Occupational Therapist/Occupational Therapist assist will study, evaluate and record your activities and progress, according to your plan of treatment ordered by your physician.

Some skilled services that your Occupational Therapist will provide to you are as followed, but not limited to:

  • Instruction and evaluation of the safe use of DME

  • Low vision therapy

  • Instruction of daily living activities

  • Instruction and assessment of energy conservation techniques

  • Instruct and improve safety in the home

  • Instruction of restorative exercises

  • Instruction and assessment of restoration of balance and posture

Speech-Therapist

Speech Therapy

Our Speech Therapist will help with speech delay disorders including articulation, phonology and motor speech disorders. Speech Therapist are essential for patients that have impairments as result of a stroke, brain injury, other type of condition. The Speech therapist will monitor and record your progress, according to your plan of treatment ordered by your physician.

Some skilled services your Speech Therapist may provide are as followed, but not limited to:

  • Sign language training

  • Auditory rehabilitation

  • Training of adaptive speech devices

  • Swallowing disorder therapy

  • Articulation exercises

Medical Social Worker

Our Medical Social Worker will help to provide support needed, to you and your family, to help to cope with injuries or illnesses - chronic, acute or terminal. The Medical Social worker will help to advise you, your family or caregivers, will provide education and counseling and will aide in making referrals for other services.

Some services your Medical Social Worker may provide are as followed, but not limited to:

  • Advanced Directives

  • Financial Aid application assistance

  • Assisting the family to adjust to loss or change

  • Alternate living arrangements

  • Grief counseling

  • Coordination of follow-up medical treatment

Certified Nursing Assistant/Aide

The Home Health Aide will help to monitor the patient’s condition by observing your physical and mental condition, your input and output, and your exercise and will report to the nurse any changes, etc. The Home Health aide will also support you by providing light housekeeping and providing/assisting with personal care/hygiene needs.

Some services your Certified Nursing Assistant/Aide will provide for you is as followed, but not limited to:

  • Assistance with activities of daily living

  • Monitoring of vital signs

  • Personal care assistance

  • Providing safe mobility support

  • Light housekeeping
